Responsibilities:

Assess patients with neurological or musculoskeletal disorders that may affect their activities of daily living.

Determine the rehabilitation potential and prognosis for each patient.

Provide a range of rehabilitation interventions to meet patient goals.

Educate patients and their families about the rehabilitation program and its importance.

Maintain accurate documentation of patients’ treatment plans, progress, and outcomes, including initial evaluations and re-evaluations.

Train healthcare professionals to administer home exercise programs.

Provide complete and detailed endorsement notes.

Assist the PT head in delivering lectures for Homecare Specialist training.
Qualifications:

Graduate of BS Physical Therapy.

Possess a valid and updated Physical Therapist license.
-
Completed Basic Life Support (BLS) Training.

Experience in handling neurological cases is a plus.

Strong verbal and written communication skills.

Excellent interpersonal skills and ability to build rapport with patients and their families.

Ability to maintain confidentiality.

Willing to work on a shifting schedule.
